This page contains all of the reporting forms that FReeZA organisations need in order to meet the requirements outlined in the FReeZA 2013 -2015 Guidelines and Conditions.

As set out in the FReeZA 2013 -2015 Guidelines and Conditions of Service Delivery, it is a requirement that FReeZA organisations provide the Community and Economic Participation Branch with two Outcome Reports per year that details the following outcomes:

  • Events - name and date of event, location, attendance
  • Training – details the training activities undertaken by young people
  • Committee Participation – name and age of young people, year joined
  • Committee Profile – general demographics of young people
  • Mentoring Activities – numbers of young people participating in mentoring activities (if applicable).
  • Event Review – outcomes or highlights from two events per year, including young people’s involvement in planning and staging the events.
  • Budget Acquittal – details of income and expenditure for your program submitted each financial year

FReeZA organisations will also be required to provide the following forms and reports:

  • FReeZA Event Notices to be lodged at least three weeks before an event
  • FReeZA Worker and Committee Details update
  • First Aid and security incidents (where an incident has occurred).
  • Risk Management and Occupational Health and Safety Plans (for outdoor events).

All completed forms, unless otherwise indicated below, should be signed and either posted or emailed to the Community and Economic Participation Branch as follows:

Event Forms

FReeZA Event Notice Form (Word, 174KB)
To promote your event through the FReeZA Gig Guide and helps us to gather your event information an Event Notice Form should be lodged at least three weeks before the event. [Send to FReeZA]

To lodge your Event Notice Form:
1. Click on the link to the FReeZA  Event Notice Form
2. Save it to your computer
3. Fill in the details of your event according to the instructions on the Form
4. Save the Form and attach it to an email
5. Email the completed Form to freeza@dhs.vic.gov.au

FReeZA Incident Report (Word, 165KB)
To report a security incident, a first aid incident or any other unusual occurrence during any FReeZA event. You must report any incident to the Office for Youth within one week of the incident. [Send to FReeZA.

Each Committee has the opportunity to promote their FReeZA activities on the site. Please take the time to update your Committee Profile as this is one of the most popular sections on the website. [Send to FReeZA.]

FReeZA Event Reviews
In addition, we love to hear about your FReeZA events or great bands or artists to be featured on the FReeZA website.
